Mountains Christian Academy has been established as an independent private school to enable enrolled homeschooling families to freely pursue their God given mandate to educate their own children. Mountains Christian Academy serves to mentor, encourage, and to provide support to enrolled homeschooling families towards this goal.

Enrollment in this private school also enables its members to meet the legal requirements set forth by the State of California Department of Education. While we encourage homeschooling families to file their own Private School Affidavits to establish themselves as their own private school, we also recognize that there are many benefits in joining together with other families with similar goals to enjoy the resources offered by Mountains Christian Academy.

Forms

The following forms are REQUIRED (PDF Acrobat files). Let me know if you cannot read them and I'll mail them to you via snail mail. If you are filing your own R4 Private School Affidavit, please feel free to download and customize our Word and Excel files.

1) Application Form PDF / Word (One per family) OR Renewal: PDF / Word

2) Course of Study PDF / Excel (Please complete one for each child) It's OK to make it brief and simple. Don't stress out over this--this is what you plan on starting with, and is not "set in concrete." You have the flexibility as a homeschooler to change curricula mid-year if you find something that will work better for your child's learning style.

3) Teacher Qualification PDF / Excel (Don't panic--remember that the main qualification is that you are "capable of teaching.")

4) Waiver of Health Examination (Form PM 171B) PDF only (this is the simplest way to file is to check the waiver box). Only print one page (second page is Spanish).

5) Immunization form (PM 286B) PDF only (OR waiver--the easiest way) This form needs two signatures if you are signing the waiver--one for immunizations, and just below it, another signature for the TB test waiver.

6) Waiver of Liability PDF

At end of school year, or by month, quarter, semester:

1) Attendance Record (One per child, to be turned in at the end of the year, or sooner if needed--please keep it up-to-date. Remember there is no required number of days in CA, but there is a requirement to keep track of attendance. You may teach year round or on a Sept-June schedule.)

Attendance Record: PDF / Excel

2) Report Cards/progress report (You can use this or make your own. Mountains Christian Academy is required to keep progress reports. Please complete either by quarter or semester.)
Report card for 1-8th grade Excel File: Front / Back PDF File: Front / Back
Report card for 9-12th grade Excel File: Front / Back PDF File: Front / Back

3) If you are new to homeschooling, you MAY be required to turn in monthly/occasional work samples or lesson plans if requested. Ask if you're unsure. We are not a "cover" school for truancy. While I do not strictly exclude famlies outside of the mountains, I strongly encourage you to find a local ISP rather than use ours if you are not in the area. I can help you locate a local group.

Membership in Home School Legal Defense Association is strongly advised. Although they will not help in cases involving problems due to divorce/custody issues, they have helped many homeschoolers avoid legal battles, as well has helping many who have been involved in legal battles. Currently our group discount cost is $95 per year, $190 for 2 years, ($8 per month/$24 per quarter automatic payments). Ask for an application and/or group discount number to complete the application online.

Transfering from another school?

Transfer of Records Form PDF / Word If your child is transferring from another school, public or private, please complete this form so we may contact your previous school in order to obtain your child's records.

Withdrawal Letter (Word File-customizable / PDF )
Here is a sample you can customize to send to the school you are leaving. While not required by law, this courtesy letter could save you from hassles if you are leaving a public school.

Curriculum: We do not provide curriculum, however, our lending library is large and is tailored to those who prefer the "real books" approach to learning. We can give you advice on what curriculum might work best for your child if you desire. Before you order your curriculum, check with us to make sure we don't have it available to borrow for the year--we have a full line of Saxon Math, for instance, for grades 5-12.

Lesson plans & work samples: We reserve the right to request lesson plans and work samples of our new families.

School Calendar: We are flexible on our school calendar, and accept year round and unusal school schedules. We trust you as parents that you will want the best for your child.

CHSPE (California High School Proficiency Examination) testing: At this time, we do not require this test for graduation, but we are considering phasing this requirement in for new students.

Need help with optional forms? I use these as assignment sheets, and sometimes as recordkeeping sheets, depending on how busy we are each week. The Excel files are easy to change if Excel is already on your computer. I would be happy to make minor customizations for you if you can't do it yourself.
